While you are in hospital:
- You should contact the Manager or person in charge for problems experienced during your stay. S/He can address most of your concerns whilst you are in hospital and ensure timely action
- Use the feedback form provided in your room / discharge lounge area
- If you wish to speak to someone outside the department – please see contact details below
Once you have left hospital:

Email your comments or feedback form to the Quality Management Department on email@example.com.
